Frozen shoulder: the three stages, and how long recovery really takes
Frozen shoulder (adhesive capsulitis) is a condition where the capsule around the shoulder joint becomes inflamed and tight, causing pain and steadily increasing stiffness. It often starts without any clear injury, and it is more common in people with diabetes.
Understanding its three stages helps, because the right approach changes as the condition evolves.
The three stages
- Freezing (painful) stage. The shoulder aches and becomes very painful — reaching out, dressing, and lying on that side hurt, and night pain is common. Movement starts reducing. This stage typically lasts around 2–9 months.
- Frozen (stiff) stage. Pain often settles somewhat, but stiffness peaks — combing hair, fastening a seatbelt, or reaching behind the back becomes difficult. This stage commonly lasts 4–12 months.
- Thawing (recovery) stage. Movement gradually returns. This can take a year or more.
How long does it take overall?
Honest answer: longer than most people expect. Frozen shoulder usually does get better, but full recovery commonly takes between six months and two years. Knowing this matters — it protects you from cycling through quick fixes and helps you commit to the steady work that actually shortens the journey.
What helps
- Targeted exercises and physiotherapy. Gentle, regular stretching to maintain and regain movement, progressing as pain allows.
- Pain control. Simple analgesics in the painful stage so you can keep the shoulder moving.
- Corticosteroid injection. If pain is not controlled, an injection into the joint can reduce pain and inflammation — most useful in the earlier stages.
- Hydrodilatation or arthroscopic release. Options for selected cases where stiffness remains severe and limiting.
What doesn’t help: resting the arm completely. Total rest tends to feed the stiffness.
When to see a doctor
- Shoulder pain with steadily reducing movement over weeks
- Night pain that disturbs sleep regularly
- Difficulty with everyday tasks like dressing or reaching overhead
- Any shoulder stiffness if you have diabetes — earlier assessment is worthwhile
