Hair fall: how much is normal, and when it's worth seeing a dermatologist

Dr. Rampal Ortho & Skin Clinic

Finding hair on the pillow or in the shower drain worries almost everyone at some point. The first useful fact: shedding 50–100 hairs a day is completely normal. Hair grows in cycles, and a portion of your roughly one lakh scalp hairs is always in the shedding phase. The question is whether you’re shedding more than that — or whether hair is genuinely thinning.

Shedding vs hair loss — they’re different problems

Excessive shedding (telogen effluvium) is when a stress on the body pushes many hairs into the shedding phase at once. Common triggers: high fever or illness (this was widespread after COVID), childbirth, rapid weight loss or crash dieting, surgery, severe emotional stress, and some medicines. The hallmark: diffuse shedding all over, typically starting 2–3 months after the trigger — and usually recovering on its own over several months once the trigger has passed.

Hair loss (most commonly androgenetic alopecia) is gradual thinning driven by genetics and hormones — a receding hairline or crown thinning in men, widening of the parting in women. This one doesn’t resolve by itself, but it responds to treatment, and earlier treatment preserves more hair.

Patchy loss — coin-shaped bald patches (alopecia areata), loss with scaling or itching (possible fungal infection), or loss along tight hairstyles (traction alopecia) — each needs its own approach.

What a dermatologist actually does for hair fall

  • Examines the scalp and hair-pull pattern to identify the type of loss
  • Looks for underlying causes — thyroid problems, low iron, vitamin D deficiency, and nutritional gaps are common and correctable
  • Builds a treatment plan, which can include evidence-based medicines (such as minoxidil, used under guidance), treatment of any underlying condition, and PRP (platelet-rich plasma) therapy in suitable cases to support regrowth
  • Sets honest expectations: hair responds slowly — visible change takes 3–6 months of consistent treatment

Practical habits that help meanwhile

  • Eat enough protein — hair is protein, and crash diets show up on the scalp
  • Be gentle: avoid very tight hairstyles and harsh chemical treatments
  • Wash regularly; a clean scalp does not cause hair fall
  • Be sceptical of miracle oils and supplements promising regrowth in weeks

When to see a dermatologist

  • Shedding clearly beyond the normal range for more than 2–3 months
  • Visible thinning, a widening parting, or a receding hairline
  • Bald patches, or hair loss with itching, scaling, or redness
  • Hair fall alongside fatigue, irregular periods, or other health changes
